Alberta International Medical Graduate Program Application
Supporting Documents
This section identifies a complete list of supporting documents
that are required to verify the information provided in your
online application. All sections may not be applicable to
individual applicants. Include all supporting documents that
pertain to your online application, e.g. if you have a former
surname you must provide the corresponding supporting document
shown below in the section ‘Name Change'.
Additional guidance and ‘how to' information is provided
in the following links:
Supporting documents included in your application package
as received by the AIMG Program, become the property of the
AIMG Program and will NOT be returned to you. Ensure that
you keep your originals.
Original documents that are not in English must be translated
by a certified translator and notarized. Required documents
are:
1. Alberta International Medical Graduate Program
Verification of Identity Form.
All AIMG applicants must have their identity verified and
credentials certified true by an Alberta notary public or
Alberta lawyer as an eligibility requirement by the AIMG Program.
- Please include an extra photo with your Alberta International
Medical Graduate Program Verification of Identity form.

This document describes the conditions for which all applicants
to the AIMG Program must agree.
3. Name change
If your surname or given names have been changed you are required
to provide proof with a copy of ONE of the following:
- marriage certificate translated into English (if applicable)
- “Change of Name” order issued by a Court
of Competent Jurisdiction
4. Status in Canada
You are required to provide a notarized copy of ONE of the
following:
- your Canadian Birth Certificate
- your Canadian Passport
- your Canadian Citizenship Certificate
- your Permanent Resident Card
- your Permanent Resident Visa and Confirmation of Permanent Residence
(COPR) document (only if awaiting receipt of your Permanent Resident Card)
- your Immigrant Visa and Record of Landing (IMM 1000) (only if you have
been granted permanent resident status and are awaiting receipt of your
Permanent Resident Card)
5. Alberta residency
You are required to provide ONE of the following:
- an official Alberta high school transcript indicating
your attendance for two or more years
- a transcript from an official Alberta post secondary
institution indicating your attendance there for two or
more years
- a completed AIMG Program Alberta Residency Statutory
Declaration form confirming that you have been a
continuous resident of the province of Alberta for 6 or
more weeks up to and including the AIMG Program application
deadline, sworn before an Alberta commissioner
of oaths, an Alberta notary public or an Alberta lawyer,
to which is attached copies of at least three substantiating
documents. See Alberta
Residency Requirement.
6. English language proficiency
If your first language is not English, you are required to
provide a notarized copy of ONE of the following:
- results from the “Test of English as a Foreign
Language – Internet Based Test” (TOEFL-IBT)
Note: requirement is a minimum
total score of 95, with a minimum of 25 in speaking, and
a minimum score of 25 in listening.
- results from the International English Language Testing
System (IELTS), academic version
Note: requirement is an overall
minimum score of 7.0, with a minimum of 7.0 in speaking
and listening
NOTE: Exam scores dated earlier than July
2nd, 2008 will not be accepted by the AIMG Program
7. Clinical rotations in Canada and the United States
If you were successful in a clinical elective or rotation
in a Canadian or United States university accredited program,
please provide ONE of the following:
- proof of attendance and completion
- copies of assessments
8. Clinical assistants and preceptorship programs
in Alberta (if applicable)
If you have participated in an Alberta clinical or preceptorship
program please provide copies of the following:
- Calgary Clinical Assistants Program (CCAP) – proof
of employment and copies of midterm and final assessments
in a sealed envelope
- International Medical Graduate Clinical Preceptorship
Program (IMGCP) – proof of employment and copies of
midterm and final assessments
- Other – proof of employment and copies of midterm
and final assessments
9. Other IMG support programs (if applicable)
If you have participated in other IMG support programs please
provide copies of the following:
- Medical Communication Assessment Project (M-CAPP) –
copy of completion certificate or letter stating current
enrollment in program
- Bredin Institute Health Care Centre – copy of completion
certificate or letter stating current enrollment in program
- Bow Valley College, Directions for Immigrants –
copy of completion certificate or letter stating current
enrollment in program
- Other – copy of completion certificate or letter
stating current enrollment in program
10. Participation in additional clinical programs
(if applicable)
- proof of completion of program and mid-term and final
assessments
11. Medical Council of Canada Evaluating Examination
score
- notarized copy of MCC letter of test results
12. Medical Council of Canada Qualifying Examination
Part 1 score
- notarized copy of letter of statement of results
Note: if you have completed the MCC-QEII
please provide a notarized copy of the statement of results.
13. Final Year Medical Students Registered for Medical
Council of Canada Qualifying Examination Part 1
- copy of Medical Council of Canada ‘Registration
complete and accepted' email notification
14. USMLE 1, 2, and, or 3 Scores (if applicable)
- notarized copy of letter of test results
15. College of Physicians and Surgeons of Alberta
License (if applicable)
16. Medical Practice License from any other regulatory
authority (if applicable)
17. Medical School Diploma
- notarized copy of your original medical school diploma
- notarized copy of English translation of medical school
diploma if original is not in English
18. Post Graduate Medical Training Certificate(s)
(if applicable)
- notarized copy of your original post graduate medical
training certificate
- notarized copy of English translation of post graduate
medical training certificate if original is not in English
19. Other post graduate education certificates and
diplomas (if applicable)
- notarized copy of your original post graduate education
certificate or diploma
- notarized copy of English translation of post graduate
education certificate or diploma if original is not in English

21. References
Each applicant is required to provide three current references
- the online application allows for a fourth referee in
the event one of your referees does not respond
